Summary Data Sheet - The Survey Summary Data sheet is used by a volunteer when he/she surveys a designated area. This form is required for each survey a volunteer completes.
Sighting Report Form - The Sighting Report Form is used by a volunteer only when he/she sites an endangered species during the survey. Volunteers should keep this form on them while completing the survey.
Authorization Letter - The Authorization letter is a DEP letter which requests access to privately owned lands. Volunteers should print this letter and keep it on them at all times during a site survey.
